Cost
- Adults: $16.00 per month. Requires a one-time enrollment cost of $79 at the point of enrollment and a 12-month commitment. (Upfront payment of $239 for the entire year available upon request).
- Children (14 and under): $12.50 per month. Requires a one-time enrollment cost of $49 at the point of enrollment and a 12-month commitment. (Upfront payment of $179 for the entire year available upon request).
Fees are based on the usual and customary fee at the time of service. Discounts do not apply to Invisalign. †10% discount available upon treatment plans paid in full. Monthly membership fees will be automatically drafted from your account. Plan can be canceled after initial 12-month membership with a 30-day notice.

It is the sole responsibility of the member to maximize their benefits by arranging all the appropriate appointments within the 12-month plan period. If the appointments are not used, the member will not be entitled to a refund.

The anniversary month of your plan is the month that you originally joined.

This is not a dental insurance plan, and can only be used at Exceptional Dental.

The plan is non-transferable.

The plan cannot be combined with any other dental insurance coverage.

The cost of treatment is not covered by enrollment or monthly payments.

Payments are due when services are rendered.
